The Richmond Behavioral Health Authority is seeking a talented and compassioniate Lead Registered Nurse Case Manager to join our PACT Team. This position will perform complex professional and advanced human support work in the care and treatment of patients in a public health program, planning, organizing and supervising the daily operation of medication management. Performs administrative work administering medications, providing medical assessments, ordering medications, managing medication administration records, drawing blood, managing urine screens, documenting activities, attending clinical staffing and training meetings, and related work as apparent or assigned. Performs intermediate professional work assessing client needs, developing, implementing and monitoring service delivery and assistance plans, coordinating and monitoring services with other agencies, counseling and assisting clients, maintaining records and files, preparing reports, and related work as apparent or assigned. Work is performed under the supervision of the Medical Director and PACT Supervisor. Essential Functions Leads oversight of all nursing duties on the PACT team and medication preparation to assure PACT medication management and room is in adherence with OSHA Standards. Obtains physician orders; orders medications; collaborates with pharmacy; transcribes orders. Assists with acquiring preauthorization’s of medication refills per pharmacy/insurance request. Administers medications both intramuscularly and orally. Completes medically related assessments including taking vital signs. Draws blood and performs urine drug screens; labels specimens via computer printout; administers and interprets TB tests; processes gel top blood tubes via centrifuge; assists, monitors and tracks lab results. Maintains thorough documentation of daily activities including nursing, treatment services, crisis intervention, case management, reimbursement, and productivity statistics. Attends and participates in staff meetings as required. Coordinates, schedules and ensures appropriate level of nursing services to clients to include monitoring, administration and documentation of injections, vena punctures and other nursing services; collaborates with treatment providers. Schedules Jab tests and injections; reschedules appointments missed by clients. Responds to medical and psychiatric emergency situations and calls emergency medical services for consumers and staff when warranted. Provides a minimum of 40% of services in the field. Performs essential personnel functions of weekend, holiday, after hours, and on-call work; reports to agency during agency closings, inclement weather, etc. Reports to PACT Supervisor in accordance with RBHA policy, which includes clinical and administrative problem solving, consultation, and training. Reports Communicate challenges and barriers in a solutions-focused way to PACT Supervisor gaps in care or concerns with medication oversight, specifically around adherence to timely administration of medications, labs and injections for PACT clients. Assists/monitors PACT psychiatrist in tracking needed labs and coordination of field and office appointments under the direction of the RN Supervisor. Maintains adequate nursing supplies. Maintains MARS, oversees management of refills by promoting the PACT primary care workers when PCP visits need to be scheduled, troubleshoot barriers to medication delivery by collaborating with the PACT team, hospitals, pharmacies, RICH clinic and outside providers. Maintain PACT Lab in a clean and organized manner. Assesses client service needs, capabilities and appropriateness for services; presents options and services based upon a needs assessment. Makes referrals and linkages to appropriate agencies for services; coordinates client services and treatment with multiple service providers and agencies. Refers and links clients to resources in the community for housing, dental clinics, NA and AA groups, food pantry, health clinics, primary care physicians, library for research or computer use, etc. Provides care coordination services to clients as necessary. Assists clients with activities of daily living including hygiene/bathing. Responsible for a small caseload of approximately five clients with high riskhigh-risk medical needs and concurring psychiatric illnessess and maintains required written records for treatment planning and assessment. Attends professional conferences and trainings as required. |
Position Requirements |
Knowledge, Skills and Abilities Thorough knowledge of the principles, standards, practices and techniques of professional nursing; thorough knowledge of the dangers inherent in and the precautions to be taken in the use of narcotics and other medications; thorough knowledge of the treatment, diagnostic, immunization and related medical services for medically ill patients; thorough knowledge of the rules, regulations, procedures and policies pertaining to nursing services of the detention facility; ability to accurately maintain patient medical records and prepare reports; ability to establish and maintain effective working relationships with clients, supervisors, directors, medical personnel, community partners, vendors, associates and the general public. Education and Experience Associate’s degree with coursework in nursing and moderate experience working with individuals with mental illnesses, or equivalent combination of education and experience. Leadership or supervisory experience preferred. Special Requirements QMHP-A Required Valid driver’s license in the Commonwealth of Virginia. Current license as a Registered Nurse issued by the Commonwealth of Virginia. |
