Hospital Admissions Liaison:
Facilitates appropriate inquiries and admissions for system-wide placement.
Conducts interaction with Columbine facility staff, discharge planners, doctors, and hospital staff.
This position is offsite and a majority of time is spent at area hospitals.
Requirements:
Strong communication and assessment skills, direct experience working with the geriatric population, and flexibility a must.
Current or previous experience in a healthcare facility preferred.
Degree in social services, human development or relevant field.
Strong computer skills required
Social Services / Discharge Planner:
Assists and advocates for residents and their families while at our facility.
Collaborates closely with the other facility departments as well as community resources.
This position will have a heavy focus on discharge planning and case management and the current opening is in our rehabilitation unit.
Assists residents in exercising and protecting their rights.
Attends and participates in interdisciplinary meetings to discuss a plan of care.
Follows and adheres to facility policies and procedures implementing HIPPA requirements.
Leads and participates in resident council and neighborhood meetings.
Organizes and facilitates Care Conferences with residents, family members, and staff.
Cooperates with expectations related to being a mandated reporter of abuse and/or neglect.
Offers support and counseling related to social and emotional factors and crisis intervention.
Creates individualized care plans for each resident.
Facilitates residents and family access to facility and community services.
Coordinates discharge and any needed community services upon discharge from the facility.
Assists residents and their families with resources pertaining to discharge planning.
Requirements:
Must have a Bachelor's Degree in Social Work or related human services field.
Experience in healthcare preferred.
Admissions Coordinator:
Completes tours and the inquiry process for potential residents and their families
Evaluates prospective admissions for appropriateness of placement and determination of correct level of care
Assists with the admission of residents and any required admission paperwork
Works with other company social service employees in facilitating smooth transitions of residents to and from different levels of care
All other duties as assigned
Requirements:
Have or is working toward a Bachelor's Degree in the Human Services Field, preferred
Must understand and interpret government regulations
Proficient with and has computer experience
