Job Description
Performs basic duties of patient care to adult and geriatric populations, under the guidance and direction of the Registered Nurse. Performs various housekeeping and cleaning duties to maintain a safe, clean environment. Performance expectations are defined in the competency evaluation.
Requirements
¨ Education Level: High school diploma or equivalent
¨ Certification/Licensure: Successful completion of approved Nurse Assistant course with certification
by the State of Illinois
BLS certification required within 30 days of hire
¨ Experience Requirements: Long term care or rehab experience preferred
REGISTERED STAFF NURSE - Multiple Shifts
Job Description
Provides safe and effective delivery of patient care, utilizing a systems theory approach, through assessment, goal setting, and outcome evaluation. Will practice within the parameters of the Illinois Nursing and Advanced Practice Nursing Act and Anderson Hospital policies and procedures. Practices autonomously and in accordance with professional nursing standards, providing care to primarily adult and geriatric populations. Performance expectations are defined in the competency evaluation. Rotating and PRN Shifts available
Requirements
¨ Education Level: Graduate of an accredited nursing program
Certification/Licensure: Illinois licensed or eligible. BLS certification requires
¨ Experience Requirements: Minimum of 1 year experience in telemetry/critical care preferred. It is a
Requirement to complete the following within 1 year of hire:
1. ACLS certification
2. EKG test
3. Critical Care Class/85% Passing Grade on Test
4. Moderate Sedation- Required to complete annual moderate sedation testing.
Applicant will be required to complete orientation and annual competency standards as outlined by hospital policies and unit standards.
NURSE MANAGER (GI Lab)(Full-time)(Days)
Job Description
Responsible for the overall delivery of patient care utilizing a system theory approach through assessment, goal setting and outcome evaluation. Practices autonomously and in accordance within professional nursing standards. Assist in administration functions of planning, coordination, supervision and evaluation of Endoscopy personnel and patient outcomes. The endoscopy nurse manager is a member of the Clinical Team and participates in other multidisciplinary teams to achieve and promote the Hospital's mission and goals
Requirements
¨ Education Level: Graduate of an accredited nursing program. BSN preferred
¨ Certification/Licensure: RN licensed or eligible; ACLS (to be completed in one year); SGNA preferred
¨ Experience Requirements: Five years nursing experience in related field; leadership experience preferred
CARE COORDINATOR
Job Description
Contributes to the coordination of care, treatment, rehabilitation and discharge planning of patients through the effective use of available resources, both internal and external. Consistently coordinates and collaborates with the patient, family and the patient care team, so that health care services are provided in a timely, cost effective and efficient manner. Activities include assessment, planning, implementation, coordination, monitoring and evaluation. Actively participates in the performance improvement process in conjunction with the clinical care teams.
Requirements
¨ Education Level:
Bachelor's Degree in Nursing preferred
(2 year degree may be considered)
Master's Degree in Social Work preferred
(Bachelor's Degree in Social Work may be considered if working toward MSW)
¨ Certification/Licensure:
Registered Nurse
LSW or LCSW preferred
¨ Previous experience in discharge planning and/or care coordination
Knowledge of medical terminology preferred.
Good communication skills are necessary.
Proficient computer skills required.
