Job Description
Contributes to the coordination of care, treatment, rehabilitation and discharge planning of patients through the effective use of available resources, both internal and external. Consistently coordinates and collaborates with the patient, family and the patient care team, so that health care services are provided in a timely, cost effective and efficient manner.
Activities include assessment, planning, implementation, coordination, monitoring and evaluation. Actively participates in the performance improvement process in conjunction with the clinical care teams.
