Care Coordinator (PRN) #20-0823

Education Level:
Bachelor's Degree in Nursing preferred
(2 year degree may be considered)
Master's Degree in Social Work preferred
(Bachelor's Degree in Social Work may be considered if working toward MSW)

 

Certification/Licensure:
Registered Nurse
LSW or LCSW preferred

 

 

Previous experience in discharge planning and/or care coordination Knowledge of medical terminology preferred. Good communication skills are necessary. Proficient computer skills required.

Contributes to the coordination of care, treatment, rehabilitation and discharge planning of patients through the effective use of available resources, both internal and external. Consistently coordinates and collaborates with the patient, family and the patient care team, so that health care services are provided in a timely, cost effective and efficient manner.

 

Activities include assessment, planning, implementation, coordination, monitoring and evaluation. Actively participates in the performance improvement process in conjunction with the clinical care teams.
